Medical extension mean you passed the interview and they wait your security or background results, mostly will get something after 3 months from your interview no response is that they want send something good not worry. To be sure make your wife ask MP or request GCMS notes.

Me got PPR than DM after week so you will get good news soon.

Sadly for them 6 or 7 months is nothing they consider it new application my application took 10 months for interview and 15 months until got PPR now waiting PUM , i hope it will move fast for you but you are still early to get upset as under 12 months is normal.
