I am yet to submit my HCP application as i dont have birth certificate. Can i Submit an affidavit along with tenth certificate which has parents name?

You need to do the following....
1) Affidavit
2) No trace certificate from the concerned municipality
3) If no trace certificate in other then english then translation attested by notary
4) Attach school certificate highlighting your dob
5) Attach passport as well for your 2nd proof as DOB
