My question is do I need to put references in United States or references from India under references section?

From what i heard, if one had renewed their passport at SFO, they already might have the PCC. I'm not sure how true this is.
This was the case for me and i got it within a week.

Yes, they require original passport. I didn't self attest any document. Notarised ones was just a copy of Visa, I-797 and driver's license. UPS guy offered to notarize all these in a single go. Question 8.a is my permanent address in India
Question 8.b Am living in the USA since the past 7 years, I think this will be my USA address ?

I think it was clarified above but to just to make sure, this will be the references from INDIA correct?
Here is what I am planning to send, kindly correct me if I am missing anything
1. Miscellaneous Form with colored photo
2. Personal Particulars Form with colored photo (cross signed)
3. EAD - notarised
4. US Address Proof - notarised
5. Passport Copy - self-attested
6. Original passport as well ?
7. Money Order - 28$

Thanks for replying, am planning to notarize it in UPS too. Sorry my bad, should have said OPT card which is my EAD, am not on H1B, its my 3rd and last chance next year and I have very little hopes of getting picked in the lottery. Even if my case got picked in the lottery, I am still going to move to Canada, not worth the wait for a green card in the states.Yes, they require original passport. On a side note, curious why bother if you have a EAD? Doesn't that give more freedom than a H1B?
