We went home for about 1 month. Do we have to list the address when we were at our home country?
I traveled few times to the states in the last five years. Do I need to include the address when I were in US?

For question 11, do we need to provide all the name/address of the former employer and former university?

I too am very unclear on both these questions: don't know what everyone else is filling in:

Question 11: name of contact?
Should we put in a specific person??
Which means we will have to call our old employer or educational institute and put in a persons info???

Question 10: should we include visits we made to home (~25 days) ?
