So my wife is in the process of requesting a police clearance from the Philippines, and all her application documents so far have only her last name on them, not mine, since she didn't officially change it.

Does anyone know if having my last name appear on the clearance (I think they put it there if you mention you're married and your husband's last name) will appear as inconsistent with the forms and cause them to return them to us?

Also, does requesting an NBI clearance have anything to do with civil status?
 
I think just for consistency she can just use her maiden name for the NBI clearance. I used my maiden name when I got my NBI clearance and that's what I submitted.

Yes I belive she can mention to the NBi that she's married. The civil status is mentioned in the clearance form as well. :)

Also, if the NSO record is not updated that she's married, can we still get an NBI stating married?

Well in my case I used my maiden name and my civil status was still single since my partner and I got married in New Zealand and our marriage isn't valid in the Philippines since it's same sex.

Maybe you can ask people in the 2015 Canada Family Class Applicants (Philippines VO) group in FB what they did for their NBI clearances since our situations are slightly different. Are you a member of the group already? :)
 
Hi I would like to ask if I should retain my maiden name and status as single in the Philippines for all Philippine documents even if I am already married in my present country where I am working? I went to Kuwait as single and married here under islamic rites so I retained my maiden name. When I apply for express entry (using an agency here) shall i make my civil status as married since I am the principal applicant and my husband will accompany me. Our marriage isn't reigistered in the Philippines yet.

Yes, she can still use here last name. Just put "Married" on the Status and put your Last Name.
But to be sure asked the officer at NBI coz that's what I did. I told them that I haven't officially changed my Last name to my husband surname and the officer told me to put married on the status and my husband surname on the form.

I got married and I didn't change my last name since I have a same sex marriage. I still had to put married. And when I got my nbi they even questioned not filling up the "husbands last name" even after I told them that I don't have a husband, I have a "wife" now I have my partners last name on the "husbands last name" it's annoying. So even though u didn't changed your last name, u might wanna mention that have a cover letter maybe and make it really clear to them

I see no big deal about this. I, myself, I am still using my maiden despite being married for almost a year now. I renewed my passport and I still used my maiden name - but I provided a copy of our marriage certificate to DFA as well as a copy of the certificate from CFO. I renewed my NBI last November 2016, stated my status as 'Married'.

Please note that on the certificate/clearance, there are 4 boxes for the applicant's name. These are: Family Name, Given Name, Middle Name, Husband's Surname. The first 3 names mentioned showed the same just like when I was still single. The only difference is that my Husband's Surname already appeared in that box and my status showed 'Married' :)

Did you figure it out? You are not getting the answers and I have the same issue as yours.

My wife did not change her name after getting married but when we asked for the police clearance back home, they've put her last name as my LastName and used her Maiden name in brackets.

When we filled the forms for EE, we said that we don't have any other names other that what's on our passport.

Any suggestions?
