I do not see any specific form for this. The reference to R117(1)(h) is the part of the acts/regs that allows sponsorship if the sponsor does not have relatives in Canada or parents/grandparents alive.
I'd recommend your sponsor fill out the IMM5406 (additional family information) AND provide some additional info in a short letter:
-confirm parents, grandparents, spouse, children are NOT alive (/ do not exist). If married before, document that sponsor has been divorced.
If any of these are alive, sponsor cannot sponsor you.
-confirm that none of spouse, common-law partner, child, parent or grand-parent, niece, nephew, aunt or auncle or cousin that is either a Canadian citizen or PR. (I think brothers and sisters here too)
If any of the above are Canadian citizens/PRs, she is not a 'lonely Canadian' and cannot sponsor you.
So if there are any of these (eg other brothers/sisters/cousins etc), she should confirm that they do NOT live in Canada / not citizens or PRs.
