VFS India asking for Appendix A form

Hi there,

We just got the letter from IRCC that my husbands PR has been approved and to send in the passport and 2 photos at a vac centre with VFS India.

On this letter nothing is mentioned about having to complete an Appendix A form but on the vfs site it mentions we need one and there is no link to this form.

Can anyone advise what VFS is asking for and if anyone has the link???

It's a table that asks for height and eye colour of the person submitting passport. You can do up any table with dob, uci#, passport number and those two bits of info and that's enough.
