Hello, I think the express entry website is very clear in this regard.

Firstly, they define
Primary Occupation as this: "Your primary occupation means the job you have experience in (within the last ten years) and want to
base your immigration application on, if you are invited to apply."
Secondly, the site states the following:
If your
primary occupation is
specialist physician (NOC 3111) or
general practitioner/family physician (NOC 3112), the
Medical Council of Canada must do an ECA for your primary medical diploma.
If you have a different primary occupation, another designated organization can do your assessment.
Therefore, we can conclude:
- If you possess a medical degree BUT your primary occupation is not either NOC 3111 / 3112, then WES/other organizations can do your evaluation.
- On the other hand, if you possess a medical degree AND your primary occupation is that of a medical practitioner (NOC 3111/3112), then MCC is the only one that can do your evaluation.
Two examples:
- "Doctor 1" has a medical degree, but his main occupation is that of a University Professor (NOC 4011). He is basing his express entry application on this NOC, so for this purpose he can have his degree evaluated by WES.
- (If "Doctor 1" manages to get PR and once in Canada decides to pursue a medical career, he will then need to have MCC evaluate his degree. But as you can see that's irrelevant for the entire express entry procedure!)
- "Doctor 2" has a medical degree and is a family practitioner (NOC 3112). He is basing his express entry application on this NOC, so MCC is the only organization that can have his degree evaluated.
Whether you're planning on becoming a practitioner or not afterwards in Canada is irrelevant for Express Entry purposes. This is because even if you do manage to get a PR with a medical degree, but a non-physician occupation, and later decide that you want to try being a doctor in Canada, then you will have to have MCC evaluate your degree later anyways.
