If approval is not guaranteed what could be the reason cuz I know medical is the last stage to approval why will the give a medical request you pass it and you're still denied visa I'm really confused

So if I enter canada right now, I can apply for my Study Permit at POE and start studying immediately or can I start my course only after my study permit arrives ?
