I don't have separate semester-wise certificates for the first and second year of my diploma, as it follows a semester pattern. However, I do possess a combined mark sheet for both the first and second years. If I were to submit this mark sheet for the first semester and second semester, would it cause any issues with the transcript?

If this is for WES, your university is expected to submit your transcripts. Nothing to do with your marksheets.
If this is for post ITA submission, you've to submit your ECA and you can attach your degree to the Education and language PDF. You don't need your marksheets for anything.
My Apologize, When I fill out the university transcript, they require semester-wise mark sheets. However, I have updated my mark sheets on a yearly basis. Will this create any issues?

In EE profile creation or in post ITA? Or are you trying to apply for WES?
You need to check with university on what they accept and what they don't. How would anyone help you here (unless I have understood your query incorrectly)
