I believe you can print all of them out(including the extra sheet), scan them into one pdf file and then upload. I believe you have to do print-and-scan anyway since the signature is required and it's not a file that can be electrically signed?

btw for my case, declarings of "all of the information provided above is truthful and accurate" are added blow each page of the extra sheets, with signatures and dates
So I had this issue for my ADR. I think some excellent suggestions have been made. To add to them, this is what I did:

A. I added a cover sheet with each upload. On the cover sheet, I indicated my application number and UCI (I had to upload documents for my spouse too, where the application number was the same but different UCI). I also indicated which part of the ADR I was responding to and provided a list of attached documents. For example, in my IMM5406 upload, I said,
(1) Completed and Signed IMM5406;
(2) Extra Continuation sheet containing: a. continuation of Section C (Brothers and Sisters – including half – and step-brothers and sisters)
b. Continuation of Sections A, B, and C – All names written in my native language

B. Then the actual IMM5406

C. Continuation sheet - in the header of the continuation sheet, I mentioned the principal applicant's full name, application number and UCI. I formatted the body of the sheet in the same style as that of the IMM5406 using tables in MS Word. Using subheadings to denote each section of the form that the information was relevant to. Finally, included a line with my name and date and signed the form.

Scanned all forms and uploaded to MyCIC.

Yes print this email and attach with your work permit application. This is your AOR . You can apply paper based but remember processing time is more . It’s better to apply online and it easy .
hello,

it seems to apply to BOWP online is online for express entry ? if i select the option on the application "move there" i wont allow me to apply for BOWP. Do you know if non express entry that got their application number still allowed to apply online or only paper? much appreciate

According to it, you should be able to apply online.

This is very hard to say unfortunately.

It depends on if your diabetes will cost the taxpayer $120,285 over 5 years (or $24,057 per year). This figure is known as the "Excess Demand" threshold. How much does your insulin cost? They may ask you to explain how much your treatments/medicines cost and need to see it go well under that threshold.

Medical Exam is focusing mainly on infectious diseases with serious problems affecting the population of Canada like Ebola, and other communicable diseases. Diabetes, hypertension..etc are not communicable diseases. So ignore this point submit your exams without any problem

Who are you and why would you tell someone to ignore my point? You say it "mainly" focusses on infectious diseases, but neglect to mention that there is also a focus on "excess demand" - there's a reason why your and my IME Form stated that we fall under the code "Non-EDE" - meaning, "Non-Excess Demand Exempt" and hence my reference to the $120,285 threshold.

I didn't make this up - I know people who have had procedural fairness letters issued due to severe cases of non-communicable diseases including HIV, Crohn's, diabetes and autism. You don't need to have an infectious disease just to be costly to the taxpayer, you could require significant physical treatments that would cause a breach of the excess demand thresholds.

So do not tell people to "ignore this point" when it is valid. Do not tell people that they can submit exams "without any problem" when you have no clue about the severity of their medical conditions or what the medical may find.

How did you upload an extra sheet?

I am trying to fill out the personal history again, but I don't have enough space. How do I make an extra sheet in the form? Do you guys know? @moscatojuices
You can use any website offering PDF combining service. Just create the additional table then save it as PDF. The upload both the form and the table to the website to combine them in one PDF file. Then upload the final PDF file combining both the form and the table
