Not sure why you're assuming the OP "marked" Islamabad for the office they wanted the application processed at. I didn't get that anywhere in the thread, although it seems the OP is assuming the application will be processed via Islamabad. So I'll address that.
MHa919 - you have two issues. First is that you forgot to send proof of medical with the application so it was sent back, so you need to get the application back to them with proof of medical. However, if there are issues with where you're eligible to have your application processed, that would have happened anyway - regardless of the medical proof not being submitted with the ap. The "rules" state that you are eligible to have your application processed at the embassy that represents your country of birth, your county of (legal) permanent residence, or a country in which you were legally admitted on your original entry for a period of at least one year. That means that if you are a permanent resident of the US, or you were initially admitted to the US with authorization to stay for at least one year (and you have that documentation) you are eligible to have your application processed through Buffalo. If you're not able to provide proof with the application showing that you have that type of eligibility status in the States, even if the ap is initially transferred from CPC-Mississauga to Buffalo, Buffalo would ship it off to Islamabad as soon as they determine you're not eligible to be processed though them. You cannot choose the visa office that will process your application, so if they transfer to Islamabad, you can't contest it.
That does not, however, mean you're required to move to Islamabad. You can be anywhere in the world while your application is processing at the visa office that represents your country of birth. Immigration Canada doesn't really care whether you're in Islamabad or not - and whether you can stay in the States depends on your immigration status there and what US Immigration does with it. Immigration Canada has nothing to say about it. As far as an interview - if you ap is processed in Islamabad, you'd have to attend an interview there - IF one is requested. And that depends almost entirely on whether or not you submit with your application sufficient proofs of your "Genuine Relationship". If they believe your relationship is genuine - and not just entered into so that you can obtain status in Canada - you could see an interview waived. However, it's possible that your immigration status appeal in the States (if they find out about it) could affect that assessment. They might be more inclined to believe that you only married to get into Canada, because you are at risk of being deported from the States. So be especially sure, under these circumstances, that you include lots of strong support of your genuine relationship with your spouse/sponsor. Document the development of your relationship, provide proof of all of his visits to see you, proof of as many (or all) phone calls made between you as you can, lots of photos of the two of you together, emails, chats, text messages, receipts for gifts, cards sent, letters sent, etc. Everything you can think of including letters from people who know you and can attest to your genuine, loving relationship. It might just be a blessing that your application was returned - if you didn't include strong support of this nature the first time around, now you have a second chance. You wouldn't get that again, short of an interview or an appeal hearing, if you hadn't provided enough evidence the first time and they'd actually accepted the application.
