Work permit extension - Made error on Imm5710

Hello guys,

I submitted a paper application for work permit extension on Dec 13. But i have made a error on IMM5710e.
I recently renewed my passport in canada, so under passport details i mentioned the country of issues as "Canada" rather than my country. I just figured that i have made this mistake.

Will this affect the decision on my extension? If so, How do i fix this issue?

Hi, Thanks for you reply. No i haven't got an application #. How do i send a web form? Do i have to link the application to mycic account to do that?

yes you have to link it with your cic account.
