I've tried to used the online wizard again and it leads me to this personal checklist:

This checklist is quite the same with my first checklist except of the REQUEST FOR MEDICAL EXAM. Under this is a link that made me download a pdf form... and this is what is written in the form..

REQUEST FOR MEDICAL

EXAM DETERMINE IF YOU NEED A MEDICAL EXAM

If you are visiting Canada for six months or less:
• You usually do not need a medical exam.
• You do need one if you plan to work in a job where public health must be protected.

See a list of Jobs for which you need a medical exam

If you are visiting for more than six months:

You will need a medical exam if you:
• have lived or stayed for six or more months in a row in one of these country or territory where you do not need a visa to enter Canada.
• are coming to Canada to work in a job where public health must be protected. See a list of jobs for which you need a medical exam
• are applying for a Parent and Grandparent Super Visa .

JOBS FOR WHICH YOU NEED A MEDICAL EXAM

-have lived or stayed for six or more months in a row in one of these country or territory in the one year before you come to Canada. This is true even if you are a citizen of a country .

Some temporary foreign workers will need to have a medical exam. This depends on the job you plan to do in Canada. Below are examples of such jobs. This list does not include every possible job.
1. Jobs that bring you into close contact with people at risk, such as:
• workers in health sciences
• clinical laboratory workers
• people who attend to patients in nursing and geriatric homes
• medical students going to university in Canada
• medical electives and doctors doing short-term assignments
• teachers
• domestics
• workers who give in-home care to children, the elderly and the disabled
• daycare workers

2. Farm workers from one of these countries or territories . If you are not sure if you need a medical exam, please contact the visa or immigration office

PLAN YOUR MEDICAL EXAM

If you are still not sure if you need a medical exam, check with a visa office If you do need a medical exam, your visa office where you applied, or will apply. near you. will give you instructions and a form with your file number before you see a panel physician. Your own doctor cannot do the medical exam. You must see a doctor on Canada’s list of Panel Physicians.

-since it is included in the personal checklist it means that i need to fill it up and submit in the complete application package.

i submited my aplication thru aramex last nov 7, 2013. then nov 19, i received an email from d embasy stating that they already rcvd my aplication.. they send also my UCI number and aplication number at the same time d list of aditional documents they are asking from my employer. dec 5, 2013 i mailed again d aditional documents but until now no response from them

may i know what are the additional requirements they ask you? mine was submitted online last dec 11 and the cic send me confirmation last dec 12, until now no response from the embassy also no additional requirements were ask, i think its still too early to expect,

aditional form to be filled up by ur prospectve employer in canada, income tax return information and income tax notice of asesment from ur prospective employer also

Can i know your personal checklist leomar? Under LCP ka rin ba?

pinkulotz, hinde ako under LCP under lower skilled ako NOC 6711 fa, my checklist are PC, NBI from philippines, some employment cert. iqama contract of employment, LMO, tsaka mga English translations. nag pass ka na din ba ng application mo?

-online ka nag pass right? Hindi pa ako nagpass... on the process pa lang ng LMO yung employer ko. Pero dahil punong puno ako ng positive energy sa katawan ko eh ngayon pa lang na.claim ko na. Hehe i'm still gathering data about the processing. And i'll start doing the paper works na for my application... no one knows baka mabilis ang process ngayong 2014...

yup nag pass ako online, actually ang immigration consultant ang nag prepare at nagpass ng application ko, oo sure na yan w8 mo lng, ung akin less than 2 months bago lumabas ung LMO ko, tapos may 3 friends ako na na refuse ang LMO kaya re apply sila ng employer nila, sana nga mapabilis na ang processing this yir kase gusto kona tlga umalis d2,,hehehe,,wer nga pla destination mo sa canada? at wer ka d2 sa saudi? pinkulotz

i would like to ask esp those u are under lcp(online application) applicants , because i received today an email requesting for my nbi,police clearance and KIT, Application forms: Application guide and forms sent to applicants"... does anyone knows what the latter means? na koconfuse ako kc ang sabi Application guide and forms sent to applicants e wla nman ako nareceive at ung sa check list ko dba laging may "?" na sign dun na pag ikiclick mo is lalabas ung details or meaning kung ano ung docu na hinihingi nila pero ung sa Application guide and forms sent to applicants wla syang ? so d ko mlaman kung ano ang ia upload ko doon.. ... :
