work permit application after receiving LMIA

Good day

I came to Canada on a working holiday visa which expires on June 1st. I applied for a visitor record on May 30th. I received LMIA yesterday.

My question is do I need to pay a restoration fee when I apply for the work permit?? The form I used is IMM 5710, how do I write " my status“ and which date for expiration should I write?

You need to pay for your work permit application fee. If you're not sure what forms you need, check cic website guidelines. People can apply online rather than paper.
