Work experience format for NOC 3111

Hi all

Badly in need of help for my work experience document.

I have received my ITA on 24 th march. I have doubts regarding the following.

1. How to describe the job duties and responsibilities.
2. Should it be exactly as mentioned in the CIC website.

I am an Ophthalmologist. So it will be under specialist in surgery. Can someone who has applied under NOC 3111 know of any acceptable format .

Please I wonder if you can help
I am a specialist physician and am applying under 3111 with my three years of training in residency after which I became a specialist but I am confused which job title should I use in my work letter? I read that a resident in training towards a speciality is still included in 3111 NOC
Any idea?
