Why would IRCC ask for RPRF fees before/during medical?

The answer is exactly what it says in the section you bolded - the principal applicant's family members need to have medicals done even if they aren't coming with you. This is the CIC definition of "family":
"An applicant’s closest relatives, in the context of an application to IRCC. It is defined as a spouse or common-law partner, dependent children, and their dependent children."

I assume you have dependent children and/or your children have dependent children who are not coming with you. Each one of them must have medicals done and submitted to CIC, not just you. You do not need to resubmit your medical. Your dependent children and their dependent children just need medicals submitted before the deadline.

I do not have any children. That is something surprising. This is a family Class sponsorship, being sponsored by my husband who is a PR. I do not have any dependent but got such a message. Thank you for your reply :)

Hmm, that's strange! Just curious, when did you submit your medical? Is it over a year old? If your passed medical is not over 1 year old, then I don't really know why else they could be asking for you to do a medical (...unless there are some mysterious children who claim you are their biological parent??? or maybe IRCC has made another mistake as they sometimes do)... Definitely send in a web form if you haven't already and reiterate that your account clearly states you passed your medical and it isn't expired, and see what their response is. You can attach a screenshot this page of your account to the webform, as well as attach the receipt you got from the panel physician when you went and did your medical. Also I see it's asking you for more supporting identity documents, so you might as well include those in the same web form as well and state that you've fulfilled their requests to the best of your knowledge.
