Whose name in page 4 of IMM0008 when sponsoring child

Dear Folks,

I am sponsoring my 1 yr old kid. Can anyone please tell me whose name should I mention in page 4 of IMM0008 under "Consent to release information to intended province/territory of destination". Should I mention my ('am the father) name or should I mention my kid name. I know that the signature below would be mine, hence the confusion.

since your 1 year old can't give consent, i'd assume the parent would be the one giving consent.

Thanks buddy, you are right that I have to give the consent. But, my question is more specific about whose name I should mention under the clause "consent to release information to intended province/territory of destination". This clause starts with I,................, on behalf of myself......
If the principal applicant is adult then obviously he would mention his name. But, in my case should i mention here my name or my kid's name.

I know I have to provide my signature on behalf of my kid. But, whose name to mention in above blank space.

In context: this form is filled out and submitted by the Principal Applicant.

Here is what it says on page 5:
CONSENT AND DECLARATION OF APPLICANT

Consent to release information to intended province/territory of destination
I, (first name, last name) ______ on behalf of myself and all dependants included in this application, authorize Citizenship and Immigration Canada to share the information collected in this application as well as supporting documentation with Provincial and Territorial authorities with responsibility for immigration for the purposes of the Immigration and Refugee Protection Act.

Given the context that the PA is submitting and declaring information on this form, I would say that first name / last name combination is you as guardian.
