Hi all, quick question: I was told that wherever a separate/additional sheet of paper needs to be included with the PR forms, the applicant's name and date of birth should be placed at the top of each page. This makes sense to me, especially for the immigration forms, but does this apply to the sponsorship forms as well? I am sponsoring my husband for PR, and I'm wondering if I should be including his (the applicant's) name at the top of my sponsorship forms, or if I should put my name instead, or both? Or if I should put down any other information? It will all be in the same packet, but either way I want to make sure we have the correct information at the top of each page. So what should we put down? :)

I would say yes, put his name and the form number. It's his application, even though the form is about you.

I put the sponsor's info on sponsor's forms and aplicant's info on aplicant's forms.
Hope it works.

Thanks guys. I was thinking of putting his information on all of the forms, but also adding my name to the sponsorship forms (and being clear who is applicant and sponsor). I just wasn't sure if there was a specific way others do it.

We just put both on anything extra.

Sponsor: SchnookoLoly
Principal Applicant: Mr Schnooks

Figured that way we were covering our bases.

I've added this info to my FAQ now, thanks! :) http://britishexpats.com/wiki/Spousal_Sponsorship-Canada/FAQ#For_some_questions.2C_I.27ll_have_to_include_extra_information_on_a_separate_sheet_of_paper._How_do_I_go_about_doing_this.3F
