Which medical form to upload??

Hi all,,

I have a e medical form given at the time of medicals. However , my physician said they will send me an aknowledgement form of medicals when they will send the report to IRCC.....that ineed to submit instead of the first one.

Any help please...

You will have to upload the e-medical information sheet which has the proper tracking number and your details mentioned on it. If your physician told you to wait for the one they will give you after submission to the IRCC, wait and use that one as it will have the list of exams performed and the date your medical report was submitted to the IRCC.

Here's a sample of what you need to upload:

I had the same situation. Mine sent me the new one with the updated status for all four exams. It changed to Completed in the eMedical form.
Maybe yours is going to do the same.

It is online application ,doctor said they gonna send results to CIC directly ,just to keep in ur file
