Where to upload IMM5646 is apply online?

Hi guy,

So I apply online March 1st 2019 for extended study permit, and March 25 2019 CIC request for Imm5646 form and must be submit by April 1 2019. I have old imm5646 from last year which signed in Jan 5th 2018. SO i submitted my old one on march 28 2019, because I called a lawyer and they said not able to get me in till April 3rd to have it notarized. so my application ending up got refused because I submit an old imm5646 form....

So now im starting a new application and wondering where I'm suppose to upload it to? because I dont see there a box for supporting documents that I can upload that to? Anyone please help me with this. I want to do this 2nd time right, So i dont have to spent another $150...

You dont need to notarize these forms.

You can upload it in the same section where you upload your SOP and other supporting docs. You will need to merge the pdfs I believe.

On bottom of form IMM5646 said signature of notary, please let me know i dont need to have it notarize it? So next year i dont have to call laywer and book appointment for this form again.

And thank i will merge it to one of my doc to send it.
