where to send police clearance

hi everyone I am new here and I will just ask where exactly to send (mailing address) my police clearance? I uploaded schedule A from my CIC account but I cant be able to send more files other than the form 5669 (pdf) I am thinking to send my documents by mail.
and also how to do the cover letter.
I hope someone can help me..

in my case there's only one button and only let's me upload one file, and thats the IMM5669 in pdf and nothing more, I uploaded it in my GCkey account. I thought it will let me send multiple files, but not. I was not really requested PCC only the schedule A, I just really thinkiing they will need it since I didnt send it along with my application, thought it would be a help for my application supporting my schedule A form, and it will be convenient for them. What would you recommend?
