where to send IMM1017B UPFRONT

Hi,

I am sponsoring my parents for permanent residence. My sponsorship application was approved and I received a letter indicating that my relatives permanent resident application has been transferred to London office.

It also noted that they must do an upfront medical and send the IMM1017B UPFRONT to the visa office directly.

Letter states:

Requirement for Medical Examination

If your sponsored relative(s) has (have) not already done so, your relative(s) must provide an original
Upfront Medical Report form (IMM1017B UPFRONT) signed by a Panel Physician or an E-Medical Upfront Medical Notification information sheet. Your relative(s) must complete the medical examination
and, once completed, forward this information directly to the visa office noted above along with a copy
of this letter. Your application has been transferred to the visa office noting this deficiency. Your relative
must forward the medical information directly to the visa office to allow continued processing of his/her
application. Failure to do so will result in delays and possible refusal of your relative's application.
*****************************************

BUT, there is no address on the letter and their is no mailing address on the London visa office website.

Where and how does one send the IMM1017B UPFRONT to London office?

I called CIC and they just emailed me the link to London visa office website.

You have to set up a doctor appointment with one of CIC approved panel physicians which you can find here, bring your letter with you to the doctor office: http://www.cic.gc.ca/pp-md/pp-list.aspx
After the physician exam will be over, he will upload the medicals of your parents to CIC database and provide you with a form which you can send t CIC via the case specific inquiry form: https://secure.cic.gc.ca/enquiries-renseignements/canada-case-cas-eng.aspx

My parents did setup an appointment with panel physician and took the letter with them. Medical was completed and they were provided us with the form.

Thank you for letting me know that we need to send this to CIC via case-specific enquiry. I am just confirming you mean CIC London, where the Permnent resident application is being processed..correct?

The other problem with case-specific inquiry is that it asks for the applicant's Client ID and Application#. I have my (sponsor) client ID and sponsorship application# but I do not have my parents client ID and their permanent resident application#. What do I put for client ID and application# for my parents. It is not noted on the medical form they received. The medical form given by the doctor has a UMI number for each applicant.

Your guidance is appreciated

You're welcome.
Yes, you send it to VO which ptocesses your case.
If you ever ordered GCMS notes you can find their UCI# in there. If not, you can just put down their names, date of birth and file # (its the same File# as yours as a sponsor).

Hello, I have received the same letter in the mail but for my husband living outside of Canada. He is living in Iran. I was wondering is this paper for my husband to go and fill out at one of the Panel Physicians or is it for me to go and get medical checks?

I assume he's the one being sponsored since he's in Iran, so he needs to make an appointment at his local panel physician and they will handle the documents. Some of the doctors submit electronically, some are paper. If it's paper, he'll have to mail it to his VO. When you phone for the appointment, ask what he needs to bring and how they handle it.

With the letter I had gotten in the mail did not come with the upfront letter. And I'm having trouble finding it. Also is this the same medical record that my husband had done when I first handed in my application? Because when I handed my application I also sent in his medical record. Which was in 2014.
Please give me some guidance. Thank you!!

It's almost the end of 2015, and medicals are only good for a year unless they (CIC) choose to extend them. Yes it's the exact same medical as he already did. The note is not for you as the sponsor doesn't need medicals. Not sure what you mean by "upfront letter" though. The doctor gives him the form. If he already went to that panel physician it will be exactly the same process, but this time he needs to mail the form to his VO (or submit it online through a case-specific enquiry).

AFAIK the letter should say where to send it or where to tell the doctor to upload to.
