the employer has gotten a LMO with my name on it

What is my next step
wher ein calgary alberta do i apply for the work permit
and what is required to be sent in with application

You have to apply to Seattle, which is the closest overseas office. If you don't have a US visa, and an interview is required you will be refused.

Do you have status in Canada?

If not unless the application can be made from the US as noted then application needs to be submitted and made from your country - perhaps your country is the US?
