Where do I get the Section A (form IMM 1017 EFC)?

My wife whom I am sponsoring has her medical exam on Tuesday, and I was told we need this form.

They don't have them anymore, the doctor should have all the papers.

the procedure has changed since December and you don't need a form anymore. You probably need to bring pictures, just call the doctor's office to check how many you would need ( it varies between 3 to 4 depending on offices), and it should be all good.

basically the doctors have been switched to a new system and you don't need to bring anything (expect from pictures). They still should give you a paper to add to your file to say that you have done the medicals though!

My wife just completed her medical exam in Islamabad Pakistan on Jan 9, 2013, you don't need to take any form with you, they will give you the form IMM1017 completed by the doctor.

I'm trying to arrange a medical in Sydney, but the practitioner there (Medibank) is insisting that they can't provide an IMM 1017. They also say that I need to bring a completed from with me, which will be supplied by the visa office _after_ I submit my application. This disagrees with the visa application guidelines, which say that I need to have the medical before I submit my application, and that I must provide a completed IMMM 1017 with my application.

Does anybody have any experience with medicals in Sydney or with Medibank? Any suggestions as to how to proceed would be greatly appreciated!

If you can, take the guide BEFORE the appointments and show them the guides. Sometimes they don't realize how fast CIC changes.

maybe call them again and explain that you're doing the spousal sponsorship, and not parents - for parents, and for other categories, they have to wait for instructions from CIC and the doctors won't do anything without the letter, but the spousal sponsorship is the other way round.
there is a sydney thread on the forum, maybe try and see where people there did their medicals so you can go to a doctor who knows the procedure, and save yourself a headache!

When I point them to the guide, they in turn point me to /english/information/medical/medexams-perm.asp on the CIC website, which suggests that I shouldn't apply for a medical until I've submitted my visa application. Can anybody explain why this page isn't relevant here, or is it just plain wrong?

Thanks, I've done this.
I mean physically take the guide in and show them. And also explain it's for spouse, not parents like stated above :)

For the benefit of anybody else for whom this is a problem ...

Medibank continued to insist that I need an IME number (obtained from CIC after applying for the visa), that I need to complete an Appendix C form, and that they can't supply an IMM 1017.

I contacted the CIC by email and got a very competent reply within a couple of days (extracts below). I forwarded this to Medibank, but they still weren't convinced, so I had to get CIC to contact them directly on my behalf! This did the trick and Medibank now agree that I don't need to bring any forms with me to the medical, and that they will supply the IMM 1017.

I am also an Australian citizen applying for Canadian PR. The invisible barriers are just insane, I rang Surry Hill clinic yesterday and guess what? they wanted me to bring 1017 form...so things haven't moved on at all it seems
