There was Bar Code on the email received by my other relatives who recently immigrated to Canada through Provincial Nominee. They received Appendix B as PDF attachment of the email.

But there is no Bar Code in my parents' Appendix B (in body of email). Instead of bar Code CIC mentioned the File Number.

I will follow you suggestions and submit a copy of the email I received from CIC along with the Appendix B I created in Word.

No, there will be no other separate email for dependents requesting Passport. You should mention all dependents' information along with the Principal Applicant on Appendix B. Submit passports, 2 copies of photos and Appendix B for PA & dependents.

There is no barcode on Appendix B. It has the file number and LVO name only but you can copy it to a word file.

You can also download the "Canada" logo from that email attachment and import it into the word doc where it says "For official use only".

Are you sending your parents passports to Ottawa or SVO?

Thank you for the suggestions. My parents are in back home now. So, I will sent their passports to SVO through VFS.

Is there any barcode on your parents' Appendix B?

As far as I know it is typical to submit Appendix B along with passport for immigration visa. CIC uses this form to identify for whom they should print COPR.

CIC did not ask us to submit Appendix B for my mom's passport request as well. Is it because she is widowed and there is only one person for CoPR?

Well, my mom's Appendix B had barcode below and it was sent from Singapore-VO.

At the bottom of Appendix B, it looks like below:


For official use only: F000XXXXXX | Singapore
Canada [barcode]

Remarks Date Received No. of Passports - IN Date Returned No. of Passports - OUT
 
UPDATE:

Passport was sent to VFS-Jalandhar(India) on Oct 10 and rec'd back with PR Visa/COPR on Oct 13. It was sent to Chandigarh for stamping. COPR is valid until expiry of Medical (one year from when it was done), in my case we have till june 2017.Thank you everyone and Good Luck.

Have you checked the bottom of the email?

At the bottom of the email that I received shows "[Message clipped] View entire message". When I clicked view entire message link, it shows the entire message where I found about the Appendix B info. When I printed that email in letter size paper, total number of pages were seven.

My Appendix B is same as yours except in place of [barcode] I have the File number. That mean, the file number is mentioned twice in my Appendix B, Similar to as shown below:

Then, you don't have to worry about it. The barcode is for FN anyway. When I copy the barcode and paste into MS Word/Notepad, it simply pastes "*F000XXXXXX*". I didn't even know that these app can read barcodes :o
