When is next healthcare draw?

No one knows. Same for when will there be education and trades draws, or a general CEC draw.
Although 490 is a good score for a category-based draw, NOC 13112 - Medical administrative assistants is not part of the healthcare-based draws. You can check that here:

CEC assuming you have at least a year of Canadian work experience. So you need a score above 500. Last draw was 521.
You would have to aim for a FSW or CEC draw, but 490 would be low for those categories. PNP is not an option as your work permit is expiring in 2 months.
So if spouse is PR or citizen then should have started the process a long time ago. Start ASAP.
That NOC is part of the healthcare draws, although they're very random. Last year I believe they only had like 3 or 4. None this year so far.
Make sure you are choosing the correct NOC based on your experience and duties. Medical Administrative Assistants and Physician Assistants are completely different things. If you pick the wrong one just to get an invitation, you'll eventually get your PR application rejected.
Also, depending on the province, you may need a license to work as a Physician Assistant or Midwife:
