I'm post ITA and have gone through medicals 5 days ago. I'm an auto-immune syndrome sufferer and I'm prepping myself for PFL. I wanna know when does one receive fairness letter? After medicals or right at the end after everything else(security, criminality, eligibility etc) is done?

Normally it would be sometime after the medical.

My spouse has been asked for a repeated x-ray after 3 months. But I'm the one with auto-immune syndrome. If we'll have to receive PFL, it would be after 3 months or it might come in a week or so from now onwards since we did our medicals 5 days ago?

It could be any of those. Anything is possible and it's really not possible for us to guess.

Any irregularities on chest x-rays typically are required to get a sputum test and repeat x-ray. Your case may need to be reviewed by a healthcare professional which takes longer
