What to Do...? Schedule 1

I had received Medical Exam request along with the following document to be submitted...

- New Schedule 1 Background/Deceleration Form

CHC London has written a comment in front of above line - "Corrected reflecting real location of Principal and Wife"

I have checked the original form I had submitted with the Full Docs and did not find any discrepancy or mistakes...

Please what shall I do? Shall I re-submit the same forms previously submitted with new dates and sign ?

Are you sure that you haven't mention any other addressees temp/permanent apart from what you wrote in Forms? Seems annoying but if CIC requested you, that means they found any confliction between your stated addressees. If you are sure that every thing is correct in way, then just refill the form and include a cover letter explaining that you submitted real/currect/temp/permanent location/addressees already and now again writing on demand from CIC. Hopefully it will work, this is what I would do if I was you. Wait for replies from other members as well.

Did u not keep the copies of whatever u had sent to CIC. iF YES then check once again and sent it once again.
