Suppose someone has studied a one year program and then got work permit for 12 months. What is he supposed to do after 12 months? Can he extend his work permit? Or he should come back to his home country and will never be able to go back to Canada?

A PGWP cannot be extended so either you extend your stay as short term visitor or return home. Alternatively you can assess if you qualify for one of the immigration programs for PR.
Completing study and a PGWP does not preclude you from coming back to Canada at any time subject of course to sucessfully applying for a visitor visa or even a new study permit although PGWP is a one time application. Alternatively check if eligible for a PR application.
What if I'm working (using my PGWP), and then it expires. Can I apply for a normal Work Permit from within Canada and continue working in the same job (provided that it's ok with the employer)?

It's not nearly that simple. In order to apply for a regular work permit, your employer has to obtain an approved LMIA. The LMIA is a long and expensive process with no guarantee of approval at the end. The vast majority of employers aren't willing to go through the process.
