What reasons if CIC reject PR even after Medical Pass?

I heard people said CIC might have the possiblilties to reject applicants after medical pass, but due to what reason?

I know this situation depends on different cases, but I want to get some information. Thanks.

If the applicant fails the security or background check.

Or a late tip-off about misrepresentation. It just happened to someone in the Family Class Sponsorship.
CIC called them back after issuing the visa, cancelled it and banned them for two years.

Security/Background Check issues would be the main reason if it does happen.

For example - something new must have popped up after the original processing
