What proof of relationship documents are required to claim 15 points for Siblings

Hi I have received ITA and now I have to show my sibling status to claim 15 points. I have one question.
1. On My sister's passport and my passport parents sir name have different for example my sister's passport father name mentioned as Ashok Kumar and my passport it's mentioned as Ashok kumar Sharma. So now what to do to with different last names. Same as mother name....
2. I have scan copy of her PR card, Driving license of canada and passport.

Please advice now what are the documents that I need to submit to claim those 15 points.
Is there affidavit required or I have to give clarification in LOE.....

I am in the similar situation plz let me know if husband documents can work out?
I am attaching their marriage certificate and out relationship.
