Form mean?

UCI: Unique Client Identifier
This number will become your PR Identity number and will still be your citizenship ID# too. You get this number once processing of permanent Residence outside of Canada has started.

E-Medical: Electronic medical. in the past it used to take 1 to 2 weeks for the medical paper file to reach RMO for processing them. Now it is like sending an email to another person. It is only taking few second for your medical to reach the Regional Medical Office.
