I gave additional document which they requested eventhough i uploaded previously and now they request for medical exam eventhough i submitted at the time of application.

Is this only me or anyone go through with this i am loosing my mind. they are requesting every single day and i cant take mind of it.

my status changed to action is required and they are reviewing eligibilty requirement.

That is true, GCkey accounts were hacked and only certain accounts were affected. I think you will be fine just re-upload the documents and mention this on the SOP
