What is Option C Printout from CRA?

I have started reading the permanent residency document checklist under the live-in caregiver program. One of the documents that I need to provide is Option C Printout from CRA by calling its toll free number. I am just wondering what this document is about (what states in it).
In the Generic Application Form, there is a question : Have you received your Certificat de Selection du Quebec (CSQ)? I am working in British Columbia so do I need to get CSQ? If I do, how do I apply for one?

The doc shows your earnings, deductions, income tax for the year. Btw, when u call CRA, be ready with ur SIN.

CSQ is only for Quebec.

Do we need to submit the original or zerox copy of Option C printout? They sent me the 2011 but not the 2012 because i have not filed it yet. Do I need to include the 2012 Option C printout since it said the Option C Printout of your entry. thank you for the anwer.

Keep the copy with you and send original of 2011.
