Hi, what happens when you are approved for permanent residency? Do you receive an official document that you could move to Canada with?

Then once you get to Canada, you apply for a PR card and show proof of the document they sent you?
 
If you applied Outland, you will receive your document (CoPR) in the mail. You travel to Canada with that document to `land' and become an official Permanent Resident.

Once you land, the officer will submit the application for your PR Card for you, but it must be sent to an address in Canada.

Can you "land" by driving into the US border and re-enter into Canada with the CoPR?

Yes, it's called flagpoling. Alternatively, you can make an appointment at an immigration office in Canada to "land" but apparently those appointments are backed up by weeks/months so it's faster to flagpole if you are already in Canada.
