Medical results are directly uploaded to CIC and you are only informed about that. During Interrogation the Physician may ask you if you smoke etc. Make sure no family member is around when you are being physically checked and being questioned at the same time. Insist to the family member that you are going alone there.

The PP will not contact your local medical clinics. If they are in doubt they will ask you for further examination, and hold the result until things get cleared. For my 3 year old daughter the PP heard a humming sound in heart and She referred to a specific Cardiovascular Pediatrician to assess in details. Then there was a specific format for the findings of the doctor and they were uploaded by the PP to CIC. She said she will have to show them the investigation she did and how she clarified her doubts.
In short they will ask further examination to rule out the doubts and will hold the uploading until things clear up.

I'm having the same freakout! I'm a heavy smoker and I'm worried the xray will show I'm dying or something and shatter all my dreams:/

To give you hope, I passed my meds, smoking for last 15 years, 8-10 sticks a day on avg.
Not promoting smoking or encouraging it but this should help you calm down.

Passive smokers can get lung cancer too. That's punishment for someone else's actions

well my dad did, died in just 2 weeks, and he was a fit man, horrible to watch and all because of a stupid habit, and even non smokers can get it but the probability for smokers is much higher

Well my this statement was according to the personal situation of OP. He does not want anyone in the family to know that he smokes. So his family members shall not be around, right? When Doctor says, Oh, I heard minute wheezing in the lungs, do you smoke? If he says NO he is in trouble, and if he says yes, he is not going to Canada in One piece ;D ;D ;D
