form IM295 work permit made outside canada..im not sure if i will add it to my given name or put it question no.2 have u used any other name and write it as middle name...

Hi,

I assume it's IMM1295B form that you were telling. Don't be confuse. Just write your given name and last name. If there's no middle name box don't write it anywhere. It should be shown the same as with your passport. If you made a mistake that's misrepresentation I guess
