If iam sending the attested transcript in the closed envelope sealed with university stamp to WES by myself, do I have to fill in Academic Records Request Form by the university controller and include it in the sealed envelope or not since i am sending by myself

even if you're sending the sealed envelope to WES yourself, you are supposed to get the WES request form included in the envelope...

though the form itself is not mandatory, I had not submitted it and had received my report.
