I think it is a generic form for all extensions from within Canada. Just follow the link on super visa section, and it will lead you there.

Tx. I am still not clear. You filed out the regular ext form? requesting 2 years or filled out a super visa form?

My folks have it till 2016 as well

Can somebody tell me is it possible to apply for an extension as Super visa? My parents are currently on Multiple entry visa which is going to be expired in June 2013 afterwards I would like to apply for their extension..Can i apply that as a super visa?
