Visitor visa after PGWP

hello everyone, I am applying for Visitor visa before my PGWP expires.. I filled the online questionnaire and I am asked to fill application for extend or changes in stay (FORM 5708) and Optional document Schedule 1- form 5257..

I read online that i have to fill FORM 5257, but I am not getting that form when I filled the questionnaire.. what should I do??
