Hello I’m filling up the visitor extension form, I’m waiting for some documents to apply for the PR , (spouse sponsorship)
My questions are:

-Coming on to Canadá question 1 , date of your original entry to Canada is the first time I came to Canada ever ? Can’t remember the exact date...
-on details of visit to Canada , so I choose family visit since I’m living with my husband who is Canadian resident or choose other and explain I’m waiting for documents to submit my spouse sponsor application?
-I’m documents list what they mean with proof of support exactly?

the date you entered Canada with the visa you are trying tore-new. It's in your passport (the entry stamp)

If you are planning to state dual intent family visit should be okay. At the same time you are supposed to convince the officer that you do not have any intentions to remain illegally in Canada should your application be refused (Visitor Record or PR application).... I read this when I was applying to extend my stay before I applied for PR: Subsection A22(2) states: “An intention by a foreign national to become a permanent resident does not preclude them from becoming a temporary resident if the officer is satisfied that they will leave Canada by the end of the period authorized for their stay”. Not necessary. Unless you want someone else to receive info about your application.

"Use this form if you want to allow Immigration, Refugees and Citizenship Canada (IRCC) to release your personal information to someone you choose. The person you choose will be able to get information on your case file, such as the status of your application."

I’m question #7 current territory of residence on the date (from ) I wrote my date of entry to Canada but I don’t know what to write in (to)

Current territory of residence (Canada) , status (visitor) and from date (Entry date) To (the date when your status will end)

Then on when you go further down you will come across details of visit to Canada where you will be required to indicate how long you intend to stay (From date when status is expected to end to long you intend to stay...if its 6 months then it should be 6 months from that date)

I will be extending my visitor visa 2nd time. Can I use same medicals that was done for my first extension. The medicals was done in Jan 2019. So its just 3 months passed. The first extension was approved and its valid until Apr 26, 2019. Any help will be appreciated.

Can you rephrase your question? Not sure what you mean by move to Canada while in the US? You CANNOT move to Canada on a visitor visa to live. You can visit though.
