Hey guys,

I have a question about applying for my medical here in Australia.

I need to make an appointment at this clinic: http://www.bupamvs.com.au/canadianmvs - Bupa in Melbourne.

It states on their website that I need to bring form IMM1017 with me showing the IME number. Thats from the Canadian embassy. Can I just print this out? Do I need to do it before or after I submit the application? I thought we just had to take our passports? Do I need to provide visa/passport photos?

Also, which of the following do I need:
Canadian visa medical assessment (excluding chest x-ray and pathology)$318.11
Canadian visa medical assessment including chest X-ray (excluding pathology)$457.06
Canadian visa medical assessment including chest X-ray and standard pathology$571.90

Thanks!
Canadian visa medical assessment including standard pathology (excluding chest x-ray)$432.95

I'm going to call them tomorrow but from what ive heard theyre sometimes contradictory. Just wondering what other Australian applicant have done. Thanks!

Hi

The IMM1017 was an old form from before the eMedical system; it is no longer required.

Some offices require people to bring photos and some don't. You need to call and ask.

You are required to do a full medical including x-ray and pathology
