VISA EXTENSION FROM WITHIN CANADA AS AN INTERNATIONAL WORKER

Hello knowledgeable people.
I landed in Canada in Dec. 2019 and haven't had a chance to leave Canada. I came as a student and now I am on a work permit. My study visa expired in 2021 Nov. now I have to apply for Work Visa. I am having issues in filling the form IMM 5257e.
1. Am I filling the correct form?
2. What do I use a National Identity Document? (Home country document or Canadian Identity document)
3. Education history.
4. Employment history.

To sum up I need help with the PAGE 3 of the form.

You need to apply for a new TRV, not an extension.
