It should say at the top “Application of for Certificate of Fact” and “Power of Attorney” on the bottom. You check the box next to “Certificate of Fact on Entry and Departure and indicate how many copies you want.
Regardless, you do not need this entry/departure record at all. I successfully obtained my certificate, and the documents that I prepared were listed below:
1. Application form for criminal records,
2. Power of attorney (notarized by the consulate office (of Korea) in your country of living),
3. Passport page, and alien registration card copy,
4. Current country visa & job-related documents (to prove with evidence that we are currently out of Korea: This is optional and based on my own idea)
5. Application for the Background check (This will not be necessary; but, you will indeed have to fill this, when you try to obtain the #2 (PoA)).
Please check the attached files (1,2 and 5) for your reference (which could be helpful) and possibly directly be used. Keep in mind that I NEVER submitted the entry/departure record. And even, I sent only the scanned version of these five documents to my friend, and she took the printed copy (by herself) to the police station and got the PCC. You do not even need to POST from wherever you live!
