VAC Consent Form. Do I suppose to fill it out?

Dear experts,

I have received a a message from CIC for TRV finalizing process. The message gave me the following requirements:
Please provide your passport or travel document, with a copy of this letter.
• There must be at least one blank page in your passport.
• Your passport must be valid for the duration of your expected length of stay in Canada.
• Please note that we cannot issue a visa beyond the expiry date on the passport.

However, in the CIC website it says I have to provide VAC consent form.

Just heads up, I am sending my passport to Los Angeles VAC.

My question is do I have to submit that form or not. By the way, I am not a US citizen.

1. Did you look at the VAC site: https://www.csc-cvac.com/en-us/selfservice/cvac_docs_forms_checklists
2. Under visitor's the first document listed is the VAC consent.
