Used my CIC account to pay for my mother's extension of stay fee

As the title states, the receipt shows my name on in. But what should I put in the "Client date of birth" and "Client ID" fields? My DoB or my mother's?

Name - Nom: PREPRINTED WITH MY NAME (which I believe is OK since on the fine print it says "Please also note that the payer's name on this receipt does not have to be the same as the one on the application form."
Client date of birth: CURRENTLY BLANK
Client ID: CURRENTLY BLANK

I had the experience to pay for parent's visit extension and friend's visitor application using my credit card.
Although there is a e-receipt, I did not fill out the receipt. It is okay.
For CIC, he only care about if the fee has been paid rather than who paid for this application.
