For OHIP even after you land you still have to wait 3-6 months to be eligible, and even then they are quite anal about the evidence provided to prove provincial residence. The rules for health care vary by province. Ontario is the only province of which I'm aware that will not extend health benefits to outland applications until you land. BC (for example) extends health benefits to dependents (including spouses) with an application pending, and the same appears to apply for AB, SK, NS and PEI.
I had to travel across the border twice before my PR card arrived and I've traveled twice since then. I've NEVER been asked for my PR card and nobody has EVER looked at my COPR.
