(URGENT) Visitor Extension approved 1 month only but medical exam requested?

I received in my mail that my visitor visa has been 'approved' but only for 1 month!! It states at the bottom of the letter that I must undergo immigration medical within 30 days, which is exactly the same as when my new visa date would expire. I received decision on 2017/04/07.

Condition:
1. Unless authorized, prohibited from engaging in employment in Canad
2. Must leave Canada by 2018/05/07
3. Unless authorized, prohibited from attending any educational institution or taking any academic, professional or vocational training course

Remarks/Observations:
Temporary resident status maintained as per R183(6)
Must undergo immigration medical within 30 days.

Am I stuck with only the 1 month or do they want me to do the medical exam in order to be approved of a longer duration? Thanks!!!

Applied on March 2nd. The approval letter was sent to me online on April 10th.

Completing the medical will not get you a longer duration. Regardless, you need to complete the medical within the required time. Your extension was granted based on that condition.

You can apply for another extension before the 7th. What is your reason for staying in Canada?
