Hi All
I just noticed that after you fill out the form on the computer and then take a print out,the column on the top right hand corner of the first page for "OFFICIAL USE " and the page numbers found on the bottom right hand corner of each page disappear. These two only appear when you click on"PRINT BLANK FORM",but then the filled out information is missing.
How have you guys handled this?
Have you printed the Blank form and then filled it manually or how
Please advise URGENTLY
